When Should You Actually See a Podiatrist?
Not every foot ache needs a doctor. But some do — and waiting can make things worse. Here's how to tell the difference.
If you're a family medicine or internal medicine physician, foot complaints probably show up in your office more often than you'd expect. Heel pain, ingrown toenails, mysterious forefoot aches, thick nails that won't respond to anything — these are the kinds of things patients mention at the end of a visit, almost as an afterthought. And while some can be managed conservatively, many reach a point where they need the focused attention of a podiatrist.
That's where a good working relationship between primary care and podiatry makes a real difference for patients.
The Conditions We See Most Often from Primary Care Referrals
Heel pain and plantar fasciitis top the list. Most patients have already tried rest and over-the-counter insoles by the time they reach us. We pick up where conservative care left off — with imaging workup, custom orthotics, shockwave therapy, and minimally invasive fasciotomy when nothing else works.
Ingrown toenails are the second most common reason we get referrals. Patients often wait far too long, and by the time they arrive, the nail is infected and painful. A 15-minute in-office procedure under local anesthesia resolves it — they walk out the same day.
Diabetic foot screening is something every endocrinologist and primary care doctor knows is important, but the comprehensive risk stratification — monofilament testing, vascular assessment, deformity mapping, pressure point evaluation — takes time. That's a natural fit for a podiatrist who does it routinely.
We also see a steady stream of bunions and hammertoes that have progressed past the point where wider shoes and padding help. Plantar warts that haven't responded to OTC treatments. Athlete's foot that keeps coming back because there's an untreated structural component. And forefoot pain — metatarsalgia, capsulitis, suspected neuromas — where precise diagnosis changes the treatment path entirely.
What Makes the Referral Work Well
The best referrals happen when the primary care physician has already done the initial workup and conservative trial. That gives us a running start. We're not repeating steps — we're building on what you've already done.
Patients also do better when they understand why they're being referred. A simple "I want you to see a foot specialist who can give this the focused attention it needs" goes a long way. Patients trust their primary care doctor's judgment, and that trust transfers.
A Few Things Patients Are Often Surprised to Learn
Many patients don't realize that podiatrists perform surgery. When we talk about minimally invasive foot surgery, they're surprised to hear it involves smaller incisions, local anesthesia, and same-day walking. It's a different picture from what most people imagine when they hear "foot surgery."
They're also surprised by how quickly most problems can be resolved once they get to the right specialist. An ingrown toenail that's been painful for weeks is fixed in 15 minutes. Heel pain that's been grinding on for months responds to treatments their primary care office simply doesn't offer.
