Required for all new patients. Works for both typing and handwriting — 9 pages with all consents.
Physician Referral Form
For referring physicians — fax to (760) 728-0061 or give to patient.
Medical Records Release
Authorize transfer of records to or from Fallbrook Podiatry.
Before your visit: Complete the intake form and bring a valid photo ID, your insurance card, and a list of current medications. Arrive 10–15 minutes early for your first appointment.
